PROMOTIONAL DATES 11/17/08-02/20/09

  1. All contestants must be a United States Citizen, and 18 years or older to enter.
  2. November 17, 2008 thru February 20, 2009 each created account will be entered into the drawing. Only entry will be allowed per household.
  3. One grand prize winner will be chosen by random drawing by Friday, February 27, 2009.
  4. Winner will receive a $500 check; this prize is non transferable, there will be no substitution of prizes. All unclaimed prizes will be reclaimed by www.ForeverMenus.com.
  5. All contest entries must complete the online account information in full. Employees, associates, and interns and their immediate families of Forever Communications, Inc. and www.ForeverMenus.com, are not eligible to win. Members of any electronic media in the Bowling Green/Glasgow area are not eligible. The term immediate family includes spouse, grandparents, parents, siblings, children and grandchildren.
  6. www.ForeverMenus.com reserves the right to end the contest at any time without additional notice.
  7. All prizes offered in this promotion can be claimed at the www.ForeverMenus.com office at 1919 Scottsville Road in Bowling Green Kentucky.
  8. There is no purchase necessary to enter or win the contest. Void where prohibited. A copy of official rules can be found online at www.ForeverMenus.com.
  9. Winner(s) will be responsible for all local, state, and federal taxes applicable based on the value of the prizes. A 1099 form will be sent to the winner. Winner will be required to complete a W-9 Federal Tax Form upon receipt of the prize.
  10. .www.ForeverMenus.com reserves the right to make rule or contest changes without notice at any time. www.ForeverMenus.com also reserves the right to change the aforementioned contest dates without notice.
  11. If the winner is found in violation of the rules, he/she may be required to forfeit his/her prize.
  12. www.ForeverMenus.com, its representatives, agencies and employees are not responsible for any liabilities arising directly or indirectly from the award or use of the prize.
  13. As a prizewinner, you acknowledge that www.ForeverMenus.com has the right to, without further authorization, publicize your name, character likeness, photograph, voice, and the fact that you are a prizewinner for promotional purposes without financial remuneration. Grand prize winner is required to complete a waiver of liability form upon receipt of prize(s). Failure to complete will result in forfeiture of prize(s).
